Paonia Care and Rehabilitation Center, a Grace Healthcare Facility, is a licensed 60 bed skilled nursing and rehabilitation center ruraly located on the Western Slope of Colorado. We are a faith based organization and believe that in every situation we must choose to “do the right thing” for all our stakeholders, residents, and our local community. We serve the rural communities such as Hotchkiss, Crawford, Somerset, as well as Paonia, bringing skilled nursing care to the local communities. We provide services of skilled nursing, post acute rehabilitation, traditional long term care, comfort care, and respite stays to the North Fork Valley and the surrounding areas.
With over 30 years of local service Paonia Care and Rehabilitation Center accepts Medicare as well as Medicaid patients. We support our patients with Estate Planning, Medicaid Assistance, Advance Directives, End of Life Services, Hospice Programs, Post Acute Care, Rehabilitation and Dementia Care.
